How to Get Involved with Connections

Connections is a highly specialized program designed to meet the unique social needs of children with AD, as well as high functioning Autism Spectrum Disorders when appropriate. As a result, participants must meet specific criteria to qualify for the program. This process typically occurs in three phases as described below:

Phase I - Record Submission

The following documentation should be submitted:

  • documentation of diagnosis
  • Background Form (required) - Please print, complete and submit with all required documentation.
  • psychological and/or psychiatric reports
  • speech and language reports
  • IEP (if receives special education services)
  • other relevant educational records and special education documents
  • Submit documentation to:

    Connections Admissions
    Summit Educational Resources
    150 Stahl Rd
    Getzville, NY 14068

Questions, please contact Ms. Mary Pat Sperrazza at 716-629-3445 or msperrazza@summited.org.

    Phase II - Record Review

Once written documentation of diagnosis, and evaluation and educational documents are received, the packet is moved to Phase II. In Phase II, the Senior Research and Clinical Team reviews all records and makes a preliminary recommendation as to whether the child applicant appears to demonstrate characteristics consistent with AD or high functioning Autism Spectrum Disorder. If the child appears to demonstrate characteristics consistent with AD or high functioning Autism Spectrum Disorder, the packet is moved to Phase III.

Phase III - Parent Interview and Child Assessment

In Phase III, the child and child’s parents/caregivers are invited in to participate in an additional assessment to confirm the diagnosis. This additional assessment is designed to provide information to further confirm the accuracy of diagnosis. Following completion of the Phase III evaluation, the Senior Research and Clinical Team reviews the results. Upon acceptance, parents/guardians complete some additional forms and formally enroll their child in the program. If a child is determined to not meet entrance requirements, an explanation of the evaluation results and reason(s) for not being accepted will be verbally shared with parents/guardians. Written reports are generally not provided as the evaluation is conducted as a screening procedure and is part of a research project. It is not intended to be used as evidence of a formal diagnosis or classification.
